About The Position
Concordia Visiting Nurses is seeking a relationship-driven professional to join our team as a Liaison. This position supports our South Hills and Washington territories and requires regular travel throughout the South Hills region and Washington County. Responsibilities
- Build and maintain strong relationships with referral sources, physicians, and community partners
- Serve as a trusted resource on our home health services
- Educate healthcare professionals on patient eligibility and care options
- Identify opportunities to support patient transitions to home-based care
- Collaborate closely with clinical and operational teams
- Represent Concordia Visiting Nurses with professionalism and integrity
