About The Position

Haven Home Health + Hospice is seeking a motivated and relationship-driven Hospice Marketer to grow our hospice services through community outreach and referral development. This role focuses on building strong partnerships with hospitals, physicians, facilities, and community organizations while promoting compassionate end-of-life care.

Requirements

• Experience in healthcare marketing, hospice, or home health preferred
• Strong communication and relationship-building skills
• Knowledge of hospice criteria and Medicare guidelines (preferred)
• Self-motivated with ability to work independently
• Valid driver’s license, reliable transportation, and current auto insurance
• Ability to travel within the Oklahoma City metro and surrounding areas

Nice To Haves

• Experience in healthcare marketing, hospice, or home health preferred

Responsibilities

• Develop and maintain referral relationships with:
• Physicians, hospitals, and discharge planners
• Skilled nursing and assisted living facilities
• Community organizations and case managers
• Educate referral sources on hospice eligibility and services
• Conduct in-person visits, presentations, and follow-ups
• Track and manage referral pipeline and activity
• Collaborate with clinical team to ensure smooth patient transitions
• Represent Haven at networking events and community functions
• Meet or exceed monthly census and referral goals
