About The Position
The Business Development Coordinator at Landmark Home Health is a full-time position based in Pittsburgh, Bridgeville, South Hills, and surrounding areas. This role is crucial in coordinating the continuation of healthcare services for patients referred to Landmark Home Health Care Services. The coordinator will be responsible for promoting and maintaining continuity of patient care from hospitals or community settings to the home environment. This involves effective communication with physicians, hospital staff, family members, home health staff, and other resources to ensure a seamless transition for patients. In this position, the coordinator acts as a representative of Landmark Home Health Care Services, serving as an educational resource to physicians, facilities, and the community. A key responsibility includes assessing the appropriateness of patients for home care, which involves evaluating the availability of caregivers and collaborating with physicians and discharge planners to ensure safe and effective care in the home setting. The coordinator will also validate and communicate the provisions of patients' home health care and equipment benefits to patients, families, and relevant stakeholders. Additionally, the coordinator will explain Landmark Home Health Care Services' policies and procedures to patients after they choose the service. This role requires compiling, submitting, and communicating admission data and necessary documentation to intake and visiting staff, as well as communicating patient discharge needs to the visiting staff or intake department. Providing a high level of customer service to both internal and external customers is essential in this role, ensuring that all parties involved are well-informed and supported throughout the process.
