About The Position

Provide Case Management leadership and management for hospitals within a market of Lifepoint Health. The position will be responsible for management or delegation of day to day operations including directing and planning. Formulate and implement operational strategies and initiatives in the case management program. Ensure staff competencies in all areas of care management to include utilization review, transition/discharge planning, social work services. Provide specialized expertise, ensure resource alignment with system-wide initiatives, and update appropriate parties on case management strategies, initiatives, outcomes, and issues. Successfully partner with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinical variation in care.

Requirements

  • RN license required.
  • Bachelor’s Degree in Nursing or Healthcare Administration; Master's preferred.
  • Minimum four to five years management experience directing a Case Management Department in an acute care hospital.
  • Working knowledge of Care Management department functions including Joint Commission and Conditions of Participation (COP) requirements.
  • Good understanding of Medicare, Medicaid, and other third-party payment systems.
  • Ability to critically think, research solutions and challenge the status quo.
  • Advanced computer skills including Microsoft Office; especially Word, Excel, and PowerPoint.
  • Strong collaborative work style and commitment to excellence in job performance.
  • Ability to work professionally with confidential, proprietary data and information while maintaining confidentiality.
  • Excellent interpersonal skills and the ability to interact effectively and professionally with individuals at all levels, both internal and external.
  • Excellent organizational skills with attention to detail and the ability to plan, implement and manage multiple projects simultaneously within inflexible time frames.
  • Must be self-motivated and have the ability to work within the established policies, procedures and practices prescribed by the organization.
  • Proven leadership with ability to navigate conflict, develop teams, and delegate.
  • English sufficient to provide and receive instructions/directions.

Responsibilities
