About The Position
ConcertoCare Market Care Navigators provide front-office support to our clinical teams delivering care through in-home and virtual visits. Care Navigators play a critical role in managing the daily flow of information and helping our patients access their healthcare services with ConcertoCare. This is a hybrid role with occasional in-office days in our NYC office. While most duties can be completed remotely, based on the local market's needs, this role may require you to commute to the local office as needed. The Market Operations Manager will set schedule.
