About The Position

MAJOR FUNCTION: Responsible for the day-to-day Social Work operations of each care center, including oversight of interdisciplinary rounds as needed. Responsible for staff mentoring and professional development, including recognition and counseling of all social work staff. Tracking and monitoring outcomes in collaboration with Department Director. Systematically assesses concerns of patients and visitors. Uses assessment data to identify problems and facilitate the resolution of problems. Directs patient rights efforts that are mandated by law. Develops and implements the Service Excellence training program.

Requirements

  • Master’s degree in social work from an Accredited College required.
  • At least 5 years Acute Hospital Social Work and discharge planning experience.
  • Experience working with Hospice, Home Care and Extended Care Facilities, Behavioral Health and Dialysis.
  • Current licensure as a Social Worker in New Jersey, either as an LSW (required) or LCSW.
  • Ability to handle potentially adversarial situations with composure and diplomacy, good follow-through ability.
  • Ability to work with teams to improve the patient’s Care Transitions and overall experience.
  • Capability to work as part of a team to improve care transition and overall patient experience.
  • Ability to coordinate, compile and analyze data.
  • Demonstrates excellent communication skills (oral and written) and interpersonal skills necessary to converse with families, patients, vendors, third-party payors and staff for optimum care.
