About The Position

The Manager-Social Work Grant is a Licensed Clinical Social Worker in the state of California and provides clinical, administrative, human resource, and financial management for social work services inpatient and outpatient areas. Develops, implements and maintains a comprehensive and fiscally sound social services program and orchestrates cohesive patient care practices which address the psychosocial needs of the patients across the continuum of care. Ensures departmental operations are aligned with the goals, objectives, and key initiatives of the PATH Grant Enhanced Care Management Program (ECM). Maintains working knowledge of all applicable state and federal regulations, organizational policies, and national standards of practice. Mentors and coaches staff in transforming culture, processes, and practice that follows the patient centered care model and meets the demands of cutting-edge health care delivery. Performs other duties as needed.

Requirements

  • Master’s Degree from a School of Social Work accredited by the Council on Social Work Education.
  • Three years of Social Work experience demonstrating competencies in leadership, clinical social work, administration, or social work education.
  • Current Clinical Social Work License (LCSW) issued by the State of California, or in progress.
