About The Position

The Program Manager, Quality & Care Transformation is responsible for leading clinical quality innovation and performance improvement strategy and care transformation initiatives. This role supports the team to develop and implement organization-wide performance improvement, and clinical innovation projects. Collaborates on development of metrics, data, tools and processes to develop, implement actions and monitor performance. Coordinates with clinical areas to assure ongoing accreditation compliance. Develops strong collaborative relationships with operational leaders, physicians across the organization and within the Clinically Integrated Network (CIN). Works closely with physicians, office staff, Clinical Integration Network (CIN) medical leadership, payors and administrative data vendors to assist in facilitating meeting established quality improvement study submissions and targets. This role requires experience and expertise that aligns with the autonomy and independence necessary for specific work assignments and decision-making, which is crucial for the speed and success of projects and outcomes.

Requirements

  • Bachelors Degree Health Administration Required Or Bachelors Degree Nursing Required Or Bachelors Degree Occupational Therapy Required Or Bachelors Degree Physical Therapy Required Or Bachelors Degree Pharmacy Required Or Bachelors Degree Health Information Management Required
  • 3 Years Experience or formal training and/or certification in PI or team facilitation including leading projects or system improvement teams.
  • Excellent communication, facilitation, analytical and motivational skills consistent with effective change management and innovation deployment.
  • Must be focused, results driven, and able to collaborate with and lead others.
  • Must be fluent in process improvement methods and tools to evaluate, measure capability, and redesign operational and clinical processes.
  • Candidate must be independent with data analysis, trending and understanding of statistical methods to determine impact of change.
  • Candidate will be able to interpret and display data in manner that identifies areas of opportunity and engages leaders to identify tactics for care improvement.

Responsibilities

  • Analyzes data pertinent to identify opportunities for improvement based on internal goals, external/internal benchmarking, industry standards and payor/external regulatory rating systems.
  • Interprets and reports data from multiple sources for comparison, benchmarking and performance assessment.
  • Independently seeks out appropriate and timely benchmark data.
  • Monitors data for criteria that is selected to evaluate clinical care and patient safety.
  • Able to develop reports from multiple sources to assure data is accurate and actionable for system improvement or reflective of issues being addressed.
  • Serves as liaison between stakeholders (e.g. clinical leaders, physicians, payers and Health Information Technology), to assure accurate data elements for reporting are identified to maximize reporting effectiveness and utilization.
  • Manages performance and process improvement projects, including those at the region, system, and corporate levels.
  • Utilizes PI tools proficiently to optimize outcomes to break down complex processes and workflows to understand areas for improvement, to minimize waste, and decrease errors.
  • Identifies, communicates and engages with key stakeholders required to address issues; moves projects forward according to defined goals and timelines.
  • Identified action steps required when goals not achieved.
  • Manages, facilitates, and/or assists with activities pertaining to various regulatory and/or accreditation requirements, encompassing tasks such as document provision, participation in discussions, and offering survey chaperone support.
  • Works closely with physicians, office staff, Clinical Integration Network (CIN) medical leadership, payors and administrative data vendors to assist in facilitating meeting established quality improvement study submissions and targets.
  • Develops educational materials and provides in-service training to physicians and office staff in the areas of quality improvement.
  • Creates disease management protocols with the CIN team.
  • Serves as a subject matter expert (SME) for clinical quality and aligns communication and work efforts with others for example with Provider Engagement teams and Lumeris Population Health Managers.
  • Generates or reviews and distributes regular clinical integration performance feedback reports.
  • Provides report interpretation and feedback to providers and office staff in meeting targeted quality goals.
  • Serves as an educator, communicator and resource to physicians, providers, and staff to ensure a thorough understanding of care management and QI objectives.
  • Collaborates with Leaders to develop quality and safety goals related to area of service in partnership with the Clinical Quality, Safety and Risk Managers
  • Maintains expert/leader level of competence in current Quality concepts, tools and performance measurements.
  • Serves as mentor/preceptor for new or less experienced staff in order to develop and maintain competence/expertise for other Quality team/organization staff members.
  • Consistently looks for and uses opportunities to network, compare outcomes and communicate both success and opportunities for improvement.
  • Effectively uses a wide variety of settings and formats to share outcomes, ideas and action plans including both formal and informal presentations, written materials, and media/tools (Power Point, bulletin boards, flyers, newsletters) to inform and educate staff, medical staff and community about local Quality achievements, initiatives and priorities.
