About The Position

Under the direction of the Director of Population Health, the Manager of Population Health Management (PHM) Manager is responsible for the ongoing development, implementation, and evaluation of HPSM’s PHM operational plans and programs ensuring alignment with departmental strategy. The Manager of PHM will oversee key PHM functions including population identification and assessments, alignment with delivery system supports, wellness and prevention integration, access to care, and evaluation and impact analysis.

Requirements

  • Bachelor’s degree in nursing, Public Health, Health Administration, Health Education/Promotion, Epidemiology, or closely related field.
  • Master’s degree in public health, public policy, or public administration with training in analysis, reporting and organizational management is preferred.
  • Three (3) years working in a nursing, public health, managed care, or similar team environment with increasing responsibility for project management activity.
  • Minimum of 2 years of experience managing people is required.
  • Advanced skill in personal computers and Microsoft Office products (e.g. Word, Excel, PowerPoint, Teams and Outlook).
  • Advanced verbal and written communication skills.
  • Advanced data collection, measurement, and maintenance skills; advanced skills in tracking and trending methods.
  • Advanced statistical analysis, quantitative and qualitative analysis skills.
  • Extensive knowledge of supervisory principles and practices as well as techniques and methods to organize and manage direct reports.
  • Extensive knowledge of principles, practices and trends of project management and business administration of Medi-Cal and Medicare programs.
  • Working knowledge of Population Health Management (PHM) methodologies and practices.
  • Extensive knowledge of leadership techniques.
  • Basic knowledge of NCQA Population Health Management standards preferred.

Responsibilities

  • Develop, manage, and lead a team of self-accountable professionals by modeling real, personal accountability and instilling a culture of psychological safety and accountability.
  • Support direct reports in holding themselves accountable to high performance standards and professional conduct in a clear, helpful, straightforward way.
  • Conduct one-on-one conversations with direct report(s), focused on performance, development, and career growth.
  • Ensure compliance with labor laws/regulations, support the implementation of organizational policies and procedures, and foster a positive and inclusive culture.
  • Support the development, implementation, and monitoring of the department budget, to ensure efficient resource allocation and compliance with policies.
  • Supervise maintenance, completion and appropriate documentation for PHM including PHM strategy, program descriptions, work plans, dashboards, policies & procedures, and impact analyses in compliance with NCQA standards.
  • Lead the preparation, validation, and submission of required reports related to PHM.
  • Design, implement and evaluate PHM programs that meet the needs of HPSM members and ensure regulatory requirements of accrediting agencies are met (e.g. NCQA, DHCS).
  • Oversee ongoing population assessments and data analysis to illustrate characteristics of HPSM member populations, understand member needs, identify gaps, prioritize opportunities for improvement, and design population level interventions to improve health outcomes and access to care.
  • Manage PHM services vendors, including quality monitoring, utilization & financial tracking, and contract compliance.
  • Use technical expertise to analyze complex problems and drive improvement activities.
  • Lead activities related to measuring/monitoring effectiveness of PHM programs including impact analysis, evaluations, and standardization of processes.
  • Administer health outcomes improvement initiatives around populations of focus or specific health topics aligned to department goals and objectives.
  • Lead cross functional teams and secure stakeholder support to drive integration of PHM initiatives.
  • Work collaboratively with internal stakeholders and external partners, community organizations and agencies to design and implement health outcomes improvement activities.
  • Facilitate intradepartmental PHM initiatives, and provide regular updates, reports and analyses of PHM activities and results to workgroups, committees and leadership.
  • Build and maintain collaborative relationships interdepartmentally to develop and meet organizational and departmental goals.
  • Represent PHM in audits, collaborative meetings, and community partnerships.
