We are seeking an experienced and compassionate Manager of Social Work, Spiritual Care & Bereavement to provide operational leadership, coordination, and performance oversight of all psychosocial, spiritual care, bereavement, and volunteer programs across our Home Health, Palliative Care, and Hospice services. This leadership role ensures the delivery of high-quality, patient- and family-centered support services through effective management of Social Workers, Chaplains, Bereavement Services, and Volunteer Programs. The Manager partners closely with Clinical and Operational leadership to ensure regulatory compliance, appropriate staffing, timely service delivery, and an exceptional care experience across the continuum.

Requirements

  • Master’s Degree in Social Work, Counseling, Theology, Healthcare Administration, or related field required.
  • Minimum of 3–5 years of experience in Hospice, Palliative Care, or Home Health required.
  • Prior supervisory or leadership experience required.
  • Knowledge of Medicare Hospice Conditions of Participation required.
  • Demonstrated ability to lead interdisciplinary teams in a healthcare setting.

Responsibilities

  • Provide direct supervision and leadership to: Licensed Social Workers Chaplains / Spiritual Care Providers Bereavement Coordinator Volunteer Coordinator
  • Conduct regular staff meetings, performance evaluations, coaching, and professional development.
  • Support onboarding, competency validation, and staff retention initiatives.
  • Foster a collaborative, mission-driven culture across supportive care disciplines.
  • Oversee psychosocial, spiritual care, bereavement, and volunteer services across all programs.
  • Ensure services align with individualized patient plans of care and IDT goals.
  • Monitor caseload distribution and staffing capacity to support admissions and timely service delivery.
  • Coordinate implementation of the Hospice bereavement program, including required 13-month follow-up for families.
  • Ensure effective recruitment and utilization of trained volunteers to support patients, caregivers, and organizational initiatives.
  • Participate in and support IDT meetings across service lines.
  • Partner with Nursing and Clinical Operations leadership to address barriers to patient access related to psychosocial or caregiver support needs.
  • Serve as liaison between supportive care staff and Intake/Admissions to facilitate safe and appropriate patient acceptance.
  • Ensure compliance with: Medicare Conditions of Participation State regulations Accreditation standards (CHAP or Joint Commission)
  • Monitor documentation for timeliness, completeness, and regulatory standards.
  • Participate in quality improvement initiatives, patient satisfaction efforts, and survey readiness activities.
  • Track and report key performance indicators, including: Timeliness of psychosocial assessments Bereavement outreach completion rates Volunteer utilization hours Patient and family satisfaction metrics
  • Contribute to strategic initiatives that support growth, access to care, and program excellence.
