About The Position

The Manager of Population Health is responsible for developing, managing and implementing strategies and to optimize population health outcomes. Additionally, this role will manage the case management and care coordination services of the organization ensuring quality and cost-effective interventions and outcomes for members. The Manager of Population Health plays a vital role in advancing the health and well-being of our members through strategic planning, collaboration, and program management.

Requirements

  • A bachelors Nursing degree (BSN) is required.
  • 6+ years of experience in clinical, nursing, quality management, population health, behavioral health, case management, disease management, health coaching or related experience.
  • 3-6+ years' experience in leadership or people management.
  • North Dakota (ND) Registered Nurse (RN) License or RN License valid for work in ND is required.
  • Must maintain an active and valid/unencumbered driver’s license which allows for standard driving privileges in North Dakota and Minnesota.
  • Demonstrated ability to lead, coach, and support staff to achieve team objectives and performance goals.
  • Advanced nursing expertise with the ability to apply complex clinical judgment and evidence‑based practices in diverse situations.
  • Strong problem‑solving and resolution skills, including analyzing complex issues, identifying root causes, and implementing effective solutions.
  • Ability to adapt to changing priorities, workflows, and member/patient needs while maintaining effectiveness and professionalism.
  • Proven collaboration skills with the ability to work seamlessly across teams, departments, and external partners to achieve shared outcomes.
  • Advanced interpersonal communication skills with the ability to build trust, influence stakeholders, and communicate clearly with clinical and non‑clinical audiences.
  • Equivalent combination of education, experience or training determined to be acceptable by Human Resources may be substituted, unless regulated by contract or program standards

  • Experience in Medicaid management is preferred.

Responsibilities

  • Leads and guides employees in accomplishing work objectives within the unit(s).
  • Coaches, motivates, rewards, and recognizes employees.
  • Sets clear goals and objectives, sets performance expectations, and holds employees accountable for achieving results.
  • Ensures program goals, projects, delegated services, and initiatives are implemented and meets department workflows, standards, and policies.
  • Supports the development of comprehensive population health management strategies and initiatives that support our members with complex medical needs that align with our corporate vision and goals and further growth of the organization.
  • Analyzes data to view population health trends and patterns.
  • Identifies health risks and high-risk populations.
  • Finds and recommends opportunities to improve health for populations, as required.
  • Develops, implements, and analyzes existing processes and procedures with an overall focus on streamlining and creating synergetic efficiencies within the department.
  • Ensures accurate documentation in accordance with new and existing workflows in and through appropriate and efficient/optimal use of clinical tools.
  • Collaborates in establishing productivity tools, metrics and reports for staff and teams, providing and reviewing productivity reports to appropriately manage performance trends and ensure efficiencies within the department.
  • Establishes and maintains business relationships with key internal and external stakeholders.
  • Provides guidance in developing and communicating integrated care plans, ensuring ongoing communication and collaboration between internal and external stakeholders.
  • Collaborates in the strategy of the Case Management Program and keeps management informed of status updates, feedback, and/or concerns.
  • Ensures compliance with regulatory requirements and industry standards related to population health and care management.
