Requirements

• Minimum of three (3) years experience in Care Coordination or Case Management.
• Bachelor's degree in Nursing or equivalent experience required.
• Current CA RN licensure.

Responsibilities

• Oversee a department of 28 FTEs (RNs and CCA) as the Nurse Manager of Care Coordination.
• Manage daily operations to ensure efficient and compliant departmental execution of Care Coordination goals and key performance metrics.
• Conduct staff performance management and promote performance improvement through data review and gathering.
• Participate in or facilitate meetings or projects related to departmental functions.
• Communicate the organization's goals to the staff and advocate for team issues, requests, or new ideas to leadership.
• Organize and execute all activities related to staffing and performance improvement in clinical services delivery.
• Implement Care Coordination activities according to the needs and policies of the Medical Center and affiliated medical groups.
• Ensure timely and compliant care coordination, discharge planning, utilization review, and social work interventions.
• Consult and collaborate with other managers, physicians, administration, and community-based healthcare workers regarding care management issues.
