About The Position

Responsible for overseeing all staff activities and administrative oversight in coordination with other Managers of Care Coordination and the Director of Care Coordination. This includes clinical supervision and administrative responsibilities.

Requirements

  • Master’s degree in Psychology, Social Work, or related field is required.
  • Must meet qualifications of a QMHP with 3 years’ experience as defined by Medicaid Rule 140, 141, and 132.
  • The ability to work with people from every diverse background and demonstrate cultural humility.
  • Experience working in community counseling-related services and working with children and adolescents with mental health needs is desired.

Nice To Haves

  • Experience with case management services and a familiarity with both medical and behavioral services is desirable.

Responsibilities

  • Responsible for reviewing client charts including LPHA/QMHP responsibilities as defined in Medicaid Rule 140, 141, and 132, and appropriate to your credentials.
  • Responsible for networking with collaterals to improve clinical services and support the agency’s mission, vision, and values along with program specific values.
  • Responsible for working with partners of the System of Care to build seamless relationships that will lead to better access for youth and families served.
  • Responsible for maintaining clinical documentation according to agency policy and procedures.
  • Provide clinical and/or administrative supervision to direct reporting staff members including Care Coordinators on a weekly basis or as needed.
  • Monitor staff direct service and Pathways/HFS specific expectations.
  • Educate community on System of Care and facilitate problem-solving meetings as necessary.
  • Assist in recruiting, engaging, and facilitating Community Stakeholder Council as outlined in Illinois’ System of Care initiative.
  • Review and approve time submitted and requests for time off from staff.
  • Complete staff evaluations to include performance appraisal, 90-day evaluation, and administer performance improvement plans as needed.
  • Meet on regular basis with Director of Care Coordination Services regarding issues and policies applicable to the Care Coordination and Support Organization (CCSO) program.
  • Responsible for responding to emergency/crisis situations with active clients and emergency back-up for agency clients.
  • In conjunction with other leadership, support crisis services in on-call supervisor rotation.
  • Work with Administrative Team in planning and evaluating Wraparound program and projects.
  • Responsible for the timely completion and submission of all agency administrative paperwork, billing, time reporting, expense reports, training forms, and any other administrative or agency requested reports.
  • In conjunction with other CCSO leadership, develop, monitor, and facilitate program goals including Performance Quality Improvement Initiatives and data reporting.
  • Responsible for hiring and training of new staff in direct reporting positions and providing on-going training
  • Responsible for leading the Care Coordination team-related risk management and performance quality improvement activities
  • Perform community education and speaking engagements.
  • Be available to participate in fundraising/promotional activities sanctioned by our agency.
  • Participate in leadership and all agency staff meetings.
  • Responsible for collaborating with funding sources regarding outcome measures.
  • Responsible for participating in job fairs and recruitment efforts as needed.
  • Attend workshops and training to keep clinical skills and knowledge current and to increase knowledge of program planning, evaluation, management, and legal issues.
  • Responsible for fulfilling agency requirement of training and documenting all training attended.
  • Must maintain certification in IM+CANS.
  • Responsible for maintaining current knowledge of changes that impact Care Coordinators, including attending trainings and technical assistance calls.
  • Review and revise, as necessary, agency forms relating to client and staff working procedures. Also, review and revise as necessary clinical forms, policies, and procedures.
  • Responsible for promoting collegiality at the workplace by working collaboratively with others.
  • Maintain licensure and/or certification that are available and appropriate to educational/ professional background.
  • Responsible for arranging staff and duty coverage when using time off
  • Responsible for being on call as scheduled.
  • Be available for in-office emergency response as needed.
