About The Position

The Manager, Care Coordination and Social Work position oversees the Care Coordinators and Social Workers in Pediatric, Obstetric and Transplant service lines. You will provide day to day oversight of operations and workflows to ensure an excellent patient experience and quality of care. You will provide support and education to foster an environment of growth and ongoing development for your team members. This work includes management and oversight of the Child Protection Committee to ensure the safety and well being of our pediatric patients. You will work closely with the interdisciplinary leadership team to enhance collaboration and development of strategic initiatives to enhance patient care. You will provide clinical and administrative oversight to ensure efficient patient flow as patients move through the continuum of care. You will lead your team in addressing Social Determinants of Health, identifying community resources and removing barriers to care. You will monitor metrics that ensure compliance with CMS, Joint Commission and quality standards.

Requirements

  • Master's degree in Social Work or Nursing.
  • Current NYS LCSW licensure or Current NYS RN license.
  • At least five years of inpatient care coordination/social work experience.
  • Proven team-building experience.

Nice To Haves

  • Management experience in Care Coordination and Social Work.
  • Commission for Case Manager Certification (CCM) or equivalent.
