The position manages the grant department and provides supervisory oversight to Clinical Program Planner(s). It carries the responsibility of training, oversight and overall management of grant related activities including research, evaluation, grant application, management and strategic planning.

Responsibilities

  • Researches community needs, collaborates with staff, and plans and organizes all information necessary to develop and prepare grant applications for funding opportunities as assigned.
  • Conducts necessary research for development of grant proposals for programs designed to meet community needs, to include a broad understanding of CentroMed’s existing programs.
  • Conducts health and social community needs/assets assessments and planning.
  • Obtains current health and social community needs assessments available in the community.
  • Ongoing collection and maintenance of electronic and print library of data sources.
  • Packages data and prepares reports to meet grant requirements, CentroMed quality advancement initiatives and other internal needs as appropriate.
  • Arranges meetings with appropriate staff persons to coordinate grant applications to include developing a timeline, reviewing work assignments, outlining objectives, and obtaining appropriate budgetary and financial documents for the grant application.
  • Writes draft of grant narrative that corresponds to the proposal guidelines in content and format and combines it with initial budget prepared by CFO.
  • Submits draft to SVP/Chief Population Health Officer for revisions and approval.
  • After application has been approved by the SVP/Chief Population Health Officer, obtains appropriate signatures, makes copies as indicated in proposal, and mails and/or electronically submits application to funder.
  • Generates/completes performance reports as required by external funders or partners.
  • Creates maps utilizing the UDS Mapper and/or other Mapping Software.
  • Measures process and outcome objectives for grant reporting as required by funders.
  • Assists with the planning, evaluation, and communication needs of CEO, SVP/Chief Population Health Officer, and Board of Directors as needed.
  • Represents CentroMed at community events and stakeholder meetings, as appropriate.
  • Understands and upholds CentroMed’s mission and values relating to ethics, integrity, safety, corporate responsibility and objectives.
  • Actively participates in maintaining high levels of excellent customer service internally and externally.
  • Abides by all policies and procedures set forth by CentroMed.
  • Ensures the protection and security of all personal, confidential and identifiable information in a professional and responsible manner and carries out all measures to prevent unauthorized disclosures.
  • Assists in training new employees in performing their job duties as requested.
  • Performs all duties in conformance to appropriate safety and security standards.
  • Performs other duties assigned.
  • Work must be performed on-site as designated.

Requirements

  • Master's Degree in Social Work, Public Health, Business Administration or related field. Experience may be substituted in lieu of advance degree at the discretion of the CEO.
  • Experience in research, planning, and grant writing.
  • Knowledge of Microsoft Word, Excel and PowerPoint.
  • Demonstrated writing, conceptual, and research skills.
  • Ability to work with and effectively communicate with people of many backgrounds, functioning at any operational level in the workplace.
