About The Position
Manager Home Care is responsible for the development, implementation, coordination, and management of home-based personal care services for PACE program participants. This position will provide direct and indirect supervision to all home care certified nursing assistants. This position acts as the leader in collaborating with members of the interdisciplinary team, providing the foundation for creating the interdisciplinary plan of care. He/She works to promote and sustain a culture reflective of Centra's best practices for quality and service, creates and maintains a safe home care environment, and contributes to a healthy work environment. The Home Care Manager is responsible for the development, implementation, coordination, and overall management of home-based personal care services for PACE program participants. This role provides both direct and indirect supervision of all home care Certified Nursing Assistants (CNAs) and serves as the supervising nurse under whose licensure CNAs practice. The Home Care Manager plays a key leadership role within the leadership and interdisciplinary teams, supporting the development and execution of comprehensive, participant-centered plans of care.
