Summary: The ACL Dementia Program is a new Homage program designed to: 1) improve health outcomes of adults with/or showing symptoms of Alzheimer’s disease and related dementias (ADRD); and 2) strengthen the dementia-capable home and community-based supports to effectively coordinate care and improve the quality of life of adults living with ADRD. The Manager of Dementia and Family Caregiver Programs will lead all efforts to launch the ACL Dementia program and work to ensure its success. In addition, the Manager will also oversee the work in Homage’s well-established Family Caregiver Program with the support of a Lead Specialist. This position has a wide range of administrative and operational tasks in support of Homage’s program operations. The Manager adheres to budgets, writes internal policies and procedures to adhere to contract compliance, and completes all reports.
