About The Position

The Community Health Manager leads the design and implementation of Sutter Health’s community grantmaking and investment strategy within a defined geographic area. This role oversees the full lifecycle of community health investments—from identifying local priorities and soliciting and reviewing proposals, to monitoring outcomes and evaluating impact. Guided by Sutter Health’s Community Health Needs Assessment and implementation strategy, the manager ensures that grantmaking decisions are strategic, transparent, and aligned with organizational goals. The manager builds and sustains strong partnerships with community-based organizations, public agencies, and local leaders to support underserved communities and advance measurable improvements in community health. They draw on best practices in community health program implementation from across the nation to inform local strategies, strengthen outcomes, and support innovation in how Sutter invests in and partners with communities. The manager also participates in the development of Sutter Health’s tri-annual Community Health Needs Assessment, ensuring that community voice and local insights inform system priorities and investment strategies. Additionally, the manager contributes to internal and external communications that highlight community health activities, partnerships, and impact stories to strengthen awareness and engagement across Sutter and the communities it serves. Working collaboratively across Sutter’s operating units and the External Affairs organization, this role connects system resources with community expertise to expand access to care, strengthen behavioral health, and grow the local health workforce. The Community Health Manager may oversee staff and resources that support grant administration, community benefit and regulatory compliance, and partnership engagement. This position plays a key role in stewarding Sutter Health’s not-for-profit mission and enhancing its reputation as a trusted community partner. The candidate selected for this role will live in and have supported the San Francisco, Marin and Sonoma County communities.

Requirements

  • Bachelor's: Public Policy, Public Health or Public Administration, or a related field
  • 8 years recent relevant experience
  • Must be ale to manage multiple projects and deadlines.
  • Must have a significant understanding of community-based organizations and local and statewide Foundations and government agencies.
  • Knowledge of public relations and event coordination.
  • Knowledge of Microsoft Word, Excel, PowerPoint, and Outlook software.
  • Excellent interpersonal and written and verbal communication.
  • Demonstrated ability to work independently with minimal direction.
  • Demonstrated ability to effectively work with executives and senior management teams.
