About The Position

Heartline Staffing, Strategic Talent Partners with Purple Cow Recruiting, is seeking an experienced RN or Licensed Clinical Social Worker (LCSW) for a permanent Manager of Case Management opportunity in Savannah, Georgia. The Manager of Case Management reports directly to the Case Management Director and is responsible for promoting patient-centered care through the coordination and oversight of hospital-based case management services. This role provides leadership, supervision, and administrative oversight for the case management team while ensuring compliance with hospital policies, regulatory requirements, and best practice standards. The Manager of Case Management functions as a clinical expert and subject matter resource for the team, supporting effective care coordination, patient throughput, and appropriate resource utilization. This role monitors quality of documentation, assessments, and interdisciplinary collaboration to ensure optimal patient outcomes while balancing efficiency and compliance.

Requirements

Bachelor’s Degree in Nursing OR Licensed Clinical Social Worker with a Master’s Degree in Social Work required.
Minimum of 3 years of experience in acute care clinical nursing OR hospital social work required.
Minimum of 2 years of supervisory or leadership experience in an acute care setting required.
Currently licensed as a Registered Nurse OR licensed as an LCSW or LMSW in the state of practice required.
