About The Position

The Manager-Case Management RN is responsible for managing the care of a specific patient population, ensuring a safe transition throughout the continuum of care while adhering to resource utilization and compliance with federal and other payer requirements. This role involves early assessment of transitional needs identified during hospitalization, illness, or life situations, and performing other assigned duties.

Requirements

• RN with 3 years of clinical experience
• RN with 5 years of clinical experience with Case Management experience in a hospital or payer setting
• Diploma or Associate Degree in Nursing
• Critical thinking skills, communication, organization, interpersonal and computer skills
• Problem solving skills and knowledge of governmental regulations
• Licensure: RN; CCM; ACM
