About The Position

Hospital Sisters Health System (HSHS) is seeking a Manager-Care Management to join our team. Implements an organized, hospital-wide care management program. Promotes an interdisciplinary process that evaluates a patient's progress from admission through discharge. Ensures that patient's care and plans for discharge are provided in the appropriate setting and in a timely, cost-effective manner. Variations from expected outcomes are analyzed and information shared to further improve plan for discharge, utilization or resources, and quality of patient care. Requirements

  • Bachelor’s degree in Nursing or Social Work is required.
  • In lieu of bachelor’s degree, diploma or an associate’s degree in nursing or in relevant clinical field with a minimum of 3 years of case management experience may be considered.
  • 3 years leadership experience in a progressively responsible role is required.
  • 2 years of care management experience is required.
  • Licensed Registered Nurse (RN) or Licensed Social Worker (LSW) in the state of practice is required.

Nice To Haves

  • Master’s degree in Nursing or Social Work is preferred.
  • Case Management certification (American Case Management Association (ACM), Commission For Case Manager Certification (CCM), Registered Nurse Board Certified (RN-BC) is preferred.
