About The Position

Under the supervision of the Director Care Management, the Manager Care Coordinator will provide clinically oriented management of daily operation for the social services programming division. The manager will share 24/7 responsibility/accountability for the daily operations within the care management department among other department leaders across the continuum of care. This position will support the coordination of care for inpatients and outpatients within the system. The manager uses depth and breadth of clinical management knowledge to ensure the provision of high quality, safe, and cost-effective patient care. The target population for services includes, but is not limited to, individuals with severe and persistent mental illness. This position will also deal with acute medically complex patients requiring intense care coordination to prevent a readmission, many of whom also experience lack of basic needs such as food, housing, and other resources.

Requirements

Requires a Master's degree in Social Work.
Must have a minimum of five (5) years of progressively responsible care management related experience that demonstrates a strong understanding of the required knowledge, skills and abilities.
Requires prior supervisory/management experience, preferably within a healthcare or care management environment.
Requires an LMSW (Licensed Master Social Worker) or LCSW (Licensed Clinical Social Worker).
Requires BLS. A BLS card obtained through an approved American Heart Association (AHA) training center is preferred.
Ambulatory Care Management: Must have a valid fingerprint clearance card issued by the Arizona Department of Public Safety OR submit a completed Affidavit at time of hire. Must complete fingerprint clearance card application within seven (7) days of hire date. Must present actual card within ninety (90) days of applying for fingerprint clearance card.
Must have the ability to supervise and coordinate the work of subordinate personnel in a manner conducive to full performance and high morale.
Must be approachable, people oriented, and a good listener.
Must be a change agent able to develop, implement and maintain policies and processes.
Requires the ability to be a creative, independent thinker.
Must exhibit a high degree of professionalism, customer service, and enthusiasm, combined with the ability to build relationships with business and key stakeholders.
Must possess solid analytical and problem-solving skills along with the ability to utilize the appropriate management techniques to plan, organize, control and coordinate activities.
Must be skilled in motivating and engaging others and providing educational presentations.
Knowledge of mental health services required.
Must have an awareness of and sensitivity to the cultural diversity of the population served.
Requires the ability to read, write and speak effectively in English.
