About The Position

Requirements

• Licensed as a Registered Nurse in New York State.
• Bachelor's Degree in a relevant field.
• A minimum of five (5) years of previous case management or discharge planning experience in an acute care setting.
• Knowledge of regulatory requirements related to acute care discharge planning.

Responsibilities

• Provide day-to-day management of RN Care Managers in the department.
• Oversee operational functions including hiring, training, and evaluating staff.
• Represent the department on assigned committees and work groups.
• Develop and implement workflows, policies, and procedures.
• Audit performance and ensure compliance with regulatory standards.
• Serve as the point person for problem escalation and troubleshooting for staff.
• Collaborate closely with Utilization Management and Appeals and Denials staff.
