About The Position

A new role at Delta, the Manager - Care Coordination provides clinical leadership and program management expertise to lead a strategy and program to support Care Management for Delta employees. This position is based in Delta General Offices in Atlanta, GA and reports into Delta's Director of Health Services.

Requirements

• Certified Nurse Case Manager
• 5 or more years of nurse case management experience with at least 3 of those years in Workers' Compensation and/or Disability/Leaves
• 3 or more years of leadership and managerial experience
• Broad clinical operations or nursing knowledge in the ambulatory care setting
• Proven experience managing outcomes and programs in a leadership capacity overseeing coordination between multiple departments or services including, or similar to, employee health, occupational health and/or ambulatory care settings.
• Proven experience with documenting processes to identify gaps and recommend opportunities for improvement
• Proven strong decision-making, problem-solving, project management and communication skills and demonstrate sound, independent judgment.
• Advanced ability to communicate to senior executives, front line employees and large groups
• Proficient in Microsoft Office applications and electronic medical record software.
• Consistently prioritizes safety and security of self, others, and personal data.
• Embraces diverse people, thinking, and styles.
• Possesses a high school diploma, GED, or high school equivalency.
• Is at least 18 years of age and has authorization to work in the United States.

Nice To Haves

• Experience working with Lean Management Systems.
• Strong preference for candidates with a certification from American Academy of Ambulatory Care Nursing (AAACN).
• Strong preference for candidates with current certification from the American Board for Occupational Health Nurses (ABOHN).
• Working knowledge of employee health, communicable illness management, infectious disease, travel health, and/or public health as it impacts a global employer.

Responsibilities

• Drives leadership and excellence in the care and experience of Delta employees experiencing a leave of absence from work relating to an on-the-job injury or disability.
• Oversees processes and system improvements to ensure high quality care management for employees across the Workers' Comp and Leaves ecosystem at Delta.
• Partners closely with multiple stakeholders, including Delta's Wellness Team (GHW), other Delta business units, the Workers' Compensation and Disability Third Party Administrator, Delta Occupational Health clinic vendors, Nurse Case Managers and others, all while focusing on the employees' health, wellbeing and recovery.
• Leverages expertise in Disability and Workers Compensation Case Management as well as leadership experience to document processes, build standard operating procedures, identify gaps, and implement continuous improvements to build an integrated model for care management excellence focused on the care and experience of injured and ill employees.
• Develops key clinical performance metrics to measure outcomes and ensure team's objectives are delivered to support corporate goals of coordinated, consistent experiences for Delta employees.
• Provides clinical expertise to consult on complex care reviews as needed.
• May be tasked with building a team if applicable and providing leadership and oversight of direct report(s).
• Supports the Wellness team in creating an environment that improves employee health outcomes and the overall wellness of our people.
