The MACT Team Specialist will function as a Case Manager on the MACT Team, while working with medical providers to address treatment. Coordinates monitors and provides direct services in a community setting for consumers. Works as a member of a clinical team to assess consumer needs. Participates in the development of Service Plans, provides/coordinates services related to the Service Plan. Acts as a consumer advocate, coordinates services and discharges plans, and assists in crisis intervention. This person will provide after-hours coverage and support to all persons being served on the team.
