About The Position

Position Summary Flagship Health is seeking a dedicated and detail-oriented Licensed Vocational Nurse (LVN) to support our clinical team in delivering high-quality, patient-centered care. The LVN will report directly to the Practice Administrator and work under the clinical supervision of a midlevel provider (NP/PA) or Physician (MD/DO). This role is integral to supporting Annual Wellness Visits (AWVs), assisting with acute triage, and contributing to quality and HEDIS performance metrics. The ideal candidate is organized, adaptable, and committed to excellence in preventive care and population health.

Requirements

  • Current Licensed Vocational Nurse (LVN) license in the applicable state.
  • CPR/BLS certification (required).
  • Strong documentation, communication, and organizational skills.
  • Ability to work in a fast-paced clinical environment and adapt to changing priorities.
  • Team-oriented mindset with a focus on patient advocacy and quality care.

Nice To Haves

  • Experience in primary care, preventive care, or value-based care preferred.
  • Familiarity with Annual Wellness Visits (AWVs) and quality metrics is a plus.

Responsibilities

  • Assist in the assessment, intake, and documentation of Annual Wellness Visits (AWVs) in accordance with CMS guidelines and Flagship Health protocols.
  • Accurately document patient histories, screenings, vitals, and required AWV elements in the EHR.
  • Administer medications and treatments as ordered by supervising providers.
  • Monitor and document patient conditions and vital signs.
  • Assist providers during clinical visits as needed.
  • Assist with triage of acute patient calls and walk-in patients, escalating appropriately to providers.
  • Gather relevant clinical information and ensure timely follow-up and documentation.
  • Support same-day workflow efficiency and patient access.
  • Assist with quality initiatives, including HEDIS and preventive care measures.
  • Support gap closure efforts (e.g., screenings, labs, immunizations).
  • Ensure accurate documentation to support quality reporting and compliance.
  • Collaborate closely with providers, medical assistants, front desk staff, and leadership to support clinic flow.
  • Assist with patient education and follow-up instructions as directed.
  • Maintain accurate, timely, and compliant medical records.
  • Adhere to infection control, safety standards, and regulatory requirements.
  • Maintain patient confidentiality and HIPAA compliance at all times.
  • Support Flagship Health policies, SOPs, and quality standards.
