Hospice of San Joaquin is the oldest not-for-profit hospice of 40 years. We serve all of San Joaquin and Stanislaus Counties. We are seeking a Dynamic skilled, self-motivated LVN - Resource Nurse who thrives in a fast-paced environment and to support our mission . Hourly range - $28.77 - $29.94 POSITION PURPOSE/SUMMARY : Presents the benefits and appropriate use of Hospice of San Joaquin Services including all services lines to the community, patients, families, physicians, SNF’s, ALFs, Memory Care Units, Discharge Planners, Social Workers, students, and anyone who may benefit from the knowledge and support. Provides consultations as requested by healthcare partners. In collaboration with the IDT assists with the completion of the hospice election, and informed consent. As a member of the IDT ensures patients who reside in facilities have comprehensive, up-to-date medical records. Participates in QAPI. A supportive resource for the Patient Access team and the Clinical team. ESSENTIAL DUTIES AND RESPONSIBILITIES: Provides education about Hospice of San Joaquin and all service lines in various environments including via phone, in patients’ homes, facilities, acute care facilities, physicians’ offices, and clinics. In collaboration with the IDT counsels patients, patient families and/or patients representatives, and community partners to ensure timely access to Hospice of San Joaquin services and service lines. Positive interaction and education strengthen community relationships and knowledge. Conducts daily facility visits to ensure clinical records are accurate, comprehensive, and assists in collaboration with the IDT and patient care needs. Uses facility chart audit tool for each patient chart audited. At the daily facility, visits offer to counsel any community member who may need information about Hospice of San Joaquin services or service lines. Develops and maintains facility community partner’s preference list including contact information, and facility-specific information detailing the needs of each facility in collaboration with the IDT. Works collaboratively with community facility partners to ensure survey preparedness as related to Hospice. Communicates daily with the Intake Department, Director of Patient Access, and IDT to facilitate an environment of teamwork. Also communicates as needed with Pacific Palliative Care. Utilizes Hospice of San Joaquin’s information systems for service contact documentation. Provides consultation outcome information to community partners in a timely fashion. Documents customer satisfaction and utilizes feedback information for continuous quality improvement. Acts as a facility contact for all education, billing, and medical records support. Maintains a professional appearance of self and work area; exhibits appropriate behavior with customers/participants and staff, i.e. respectful, courteous, supportive and professional at all times. Ensures HIPPA compliance. Additional duties as assigned by the Director of Patient Access or designee. Adhere to all Hospice of San Joaquin Policies and Procedures including the Hospice of San Joaquin Compliance manual. QUALITY ASSESSMENT/IMPROVEMENT AND UTILIZATION REVIEW: Ensures compliance with the program plan, assuring quality service and appropriate utilization of resources. Participates in Quality Assessment/Improvement and Utilization Review activities as requested by the administration. Incorporates results of AQ/AI and Utilization Review studies in nursing practice.

Requirements

  • A current license in the State of California as a Licensed Vocational Nurse.
  • One-year recent clinical experience dealing with acute care patients preferred.
  • Six months or more of home care or Hospice experience preferred.
  • Ability to set priorities, organize, and work independently.
  • Knowledge of medications, indications, dosage ranges, side effects, and potential toxicity.
  • Skill in nursing practices with understanding and knowledge of Hospice care and commitment to the philosophy and goals of the program.
  • Ability to communicate effectively verbally and in writing.
  • CPR training
  • Graduate of an accredited school of nursing in the State of California as a Licensed Vocational Nurse.
  • One year of recent clinical experience dealing with acute care patients preferred.
  • Six months or more of home care or Hospice experience preferred.
  • Knowledge of advancing chronic and life-limiting illness signs/symptoms; treatment interventions and familiarity of medications used for symptom management and comfort care is necessary.
  • Experience with Hospice services and knowledge of hospice regulatory requirements and Medicare COP guidelines is preferable.
  • Can work independently, demonstrates excellent organizational and presentation skills, is detail-oriented, customer-focused, and is able to speak persuasively in public.
  • Must possess computer skills, and the ability to learn and assimilate the clinical computer program system is required.
  • Can handle multiple projects and tasks simultaneously, is flexible, and uses time and resources efficiently to accomplish work objectives.
  • Must be able to work in a high-stress environment(s) or with customers, staff, or community who are under stress while maintaining a positive work attitude.
  • Demonstrates ability to consistently deliver on obligations and promises to customers.
  • Possess reliable transportation, current California driver’s license, and automobile insurance which meets California state standards at hire, at each renewal period and maintain a safe driving record.
  • Possess a license to practice as a Licensed Vocational Nurse in the State of California.

  • Ability and willingness to drive own vehicle, have a valid California driver’s license and have automobile insurance that meets State standards.
  • All employees, volunteers, consultants, and consultant volunteers who drive need to provide Hospice of San Joaquin with evidence of insurance.
  • This is necessary to be covered on the agency’s non-owned auto policy.
  • Ability to safely maneuver equipment (wheelchairs, etc.) and assist family/caregivers with transferring and ambulating patients when necessary.
  • Implements personnel policies as they relate to nursing staff.
