About The Position

Hospice of the East Bay (HEB) is a community-based program of home care for terminally ill patients. Care is given to patients and families by an interdisciplinary group. Bereavement care is provided to the family for one year after the death. Hospice services are given regardless of ability to pay, making the agency dependent on donations and insurance billing to meet operating expenses. Coverage Areas: American Canyon, Hercules, Richmond, Oakland, Castro Valley, Walnut Creek, Pleasanton, Livermore, Antioch, Brentwood and the surrounding cities in the area. Position Summary With general direction from the Hospice Physician (or Nurse Practitioner) and under the supervision of a Registered Nurse (RN), the Licensed Vocational Nurse (LVN) provides direct nursing care and related assistance to patients and families as required by the assigned program/department. Although the LVN reports to a primary manager, HEB expects the LVN may work in more than one program or department.

Requirements

  • Must have excellent interpersonal, communication and organizational skills, knowledge of pain and symptom management, knowledge of hospice regulations.
  • Possessing the ability to establish and maintain cooperative relationships with professionals and families, able to collaboratively work as part of an interdisciplinary group, and the willingness and flexibility to be cross-trained in more than one HEB program or department
  • Current, active LVN license required; must be verified by the CA Board of Vocational Nursing and Psychiatric Technicians
  • At least one year nursing experience as LVN required; experience working in acute care and/or with end-of-life patients highly desirable
  • Current CA Driver’s License and car in good working condition required
  • Current CPR certification required

Nice To Haves

  • IV certification preferred
  • Hospice certification preferred

Responsibilities

  • Provide hospice care to terminally ill patients in their place of residence (e.g., home or facility) and in collaboration with the HEB interdisciplinary team and other medical staff as appropriate.
  • Provide support for the patient’s family.
  • Maintain communication with patient, family and facility staff as required and including education on pain/symptom management and psycho-social aspects of hospice care.
  • Attend, as requested, and present appropriate patient-related information at interdisciplinary team meetings.
  • Gather data for plan of care, implementation and revision of the interdisciplinary group care plan for each patient. For SNF patients, in conjunction with the facility plan of care.
  • Observe patients and report changes in condition to appropriate person (e.g., Case Manager).
  • Communicate with appropriate medical staff regarding patient acuity, scheduling, and pending physician communication.
  • Complete documentation timely and accurately.
  • Attend and participate in nursing staff meetings and HEB in-services.
  • Participate in the agency quality improvement activities as assigned
