Hospice of the East Bay (HEB) is a community-based program of home care for terminally ill patients. Care is given to patients and families by an interdisciplinary group. Bereavement care is provided to the family for one year after the death. Hospice services are given regardless of ability to pay, making the agency dependent on donations and insurance billing to meet operating expenses. Bruns House, located in the beautiful town of Alamo, is the only inpatient hospice facility in the Bay Area. It offers six private rooms designed to provide the highest level of compassionate hospice care. Hospice East Bay has been certified the past five years in a row as a "Great Place to Work". Position Summary – LVN, Bruns House, PM Shift, every other weekend With general direction from the Hospice Physician (or Nurse Practitioner) and under the supervision of a Registered Nurse (RN), the Licensed Vocational Nurse (LVN) provides direct nursing care and related assistance to patients and families as required by the assigned program/department. Although the LVN reports to a primary manager, HEB expects the LVN may work in more than one program or department. Home Care: Reporting to a Team Manager, the LVN assists with covering a caseload for an absent Nurse Case Manager, making home visits and coordinating patient/family needs. This program provides hospice care for patients residing at home. Long-Term Care (LTC): Reporting to a Team Manager, the LVN assists with covering a caseload for an absent Nurse Case Manager, making home visits and coordinating patient/family needs. This program provides hospice care for patients residing in facilities and requires close collaboration with facility staff. After Hours: Reporting to the Team Manager, the LVN is responsible for making home visits, identifying and coordinating patient/family care needs during weeknights and weekends. Admissions: Reporting to the Admissions Manager, the Admissions Nurse is responsible for the onsite assessment of the patient and family for appropriateness of Hospice Services. The LVN completes the initial assessment, plan of care and handles all the administrative requirements upon admission (e.g. explanation of benefits, insurance, bill of rights, DNR, etc.)
