About The Position

Requirements

• LVN license in California required; RN license preferred.
• 2-3 years of experience with progressive leadership responsibilities.
• Preferred experience in Assisted Living settings.

• Knowledge of healthcare regulations and business management.

Responsibilities

• Develop and implement wellness programs to support physical, emotional, and social well-being.
• Lead and supervise the wellness team, ensuring 24/7 coverage and staff development.
• Conduct health assessments and create personalized care plans for residents.
• Coordinate with hospitals, physicians, and other agencies to deliver optimal care.
• Oversee patient care in diverse settings, including hospice, memory care, and home care.
• Ensure compliance with CDC, OSHA, and infection control standards.
• Utilize electronic health records for documentation and progress tracking.
• Provide training for staff and engage families in care planning and wellness education.
