About The Position
At Marysville Post Acute, we are committed to providing compassionate, high-quality care in a supportive and collaborative environment. We are seeking a dedicated LVN Unit Manager to lead care coordination efforts and ensure our residents receive exceptional, personalized care.
Requirements
- Active LVN/LPN license (RN a plus)
- Bachelor’s Degree in Nursing
- Minimum 2 years of clinical experience
- Knowledge of Medicare, Medi-Cal, and Medicaid
- Strong communication, organization, and time management skills
- Ability to work independently in a fast-paced environment
- Team player with a collaborative mindset
Nice To Haves
- Experience with PointClickCare (PCC) preferred
Responsibilities
- Serve as a key point of contact for residents and families regarding care plans, therapies (PT/OT), and treatment updates
- Coordinate and oversee admissions, discharges, and referrals
- Participate in daily stand-up meetings to support team communication and continuity of care
- Monitor resident progress and adjust care plans as conditions change
- Perform utilization reviews to ensure timely and cost-effective care
- Ensure accurate, compliant, and timely documentation
- Communicate effectively with physicians, providers, and insurance/payor sources
- Support residents and families so they feel informed, safe, and confident in their care
