About The Position

The LVN Supervisor is responsible for overseeing and coordinating resident care within a Skilled Nursing Facility (SNF). This role provides clinical supervision, ensures compliance with regulatory standards, supports care planning, and facilitates effective communication among residents, families, nursing staff, physicians, and interdisciplinary team members. The LVN Supervisor promotes high-quality, safe, and cost-effective care while supporting admissions, discharges, and ongoing resident needs.

Requirements

  • Graduate of an accredited Licensed Vocational Nursing (LVN) program.
  • Active and valid LVN license required in the state of employment.
  • Minimum of two (2) years of clinical experience in a Skilled Nursing Facility (SNF) required.
  • Knowledge of Medicare, Medi-Cal, and Medicaid regulations and skilled nursing benefits required.
  • Ability to read, interpret, and apply clinical procedures, policies, and regulatory guidance.
  • Ability to clearly communicate information to residents, families, staff, physicians, and management.
  • Ability to apply basic mathematical concepts such as fractions, percentages, ratios, and proportions in clinical and staffing situations.
  • Ability to assess resident conditions, prioritize care needs, and solve practical clinical and operational problems.
  • Ability to interpret instructions provided in written, oral, diagram, or schedule form.
  • Current, valid LVN license in the state of employment (required).
  • Must maintain all required continuing education and licensure.
  • Must remain in good standing with the Department of Public Health, License and Certification Division at all times.

Nice To Haves

  • Prior supervisory or charge nurse experience in skilled nursing preferred.
  • Familiarity with PCC or similar electronic health record (EHR) systems preferred.
  • Certified Case Manager (CCM) or other supervisory certification a plus.

Responsibilities

  • Provide clinical supervision and support to nursing staff within assigned units or shifts.
  • Communicate regularly with residents and their families regarding plans of care, skilled nursing services, physical therapy (PT), occupational therapy (OT), and other treatment protocols.
  • Attend and actively participate in daily stand-up meetings, interdisciplinary team meetings, and care conferences to ensure continuity of care.
  • Answer resident and family questions regarding care, treatment plans, illness progression, and facility services to promote confidence and trust.
  • Monitor resident conditions and ensure timely interventions based on changing clinical needs.
  • Coordinate and oversee admissions, transfers, and discharge planning in collaboration with the interdisciplinary team.
  • Review, monitor, and ensure accuracy of nursing documentation and clinical records.
  • Perform utilization review activities to support appropriate, timely, and cost-effective skilled nursing care.
  • Coordinate care with physicians, therapists, ancillary service providers, financial services, and third-party payers.
  • Facilitate referrals to and from the facility as needed.
  • Promote collaboration and effective communication among residents, families, caregivers, physicians, nurses, and ancillary partners.
  • Organize, prioritize, and delegate daily work based on census, acuity, admissions, and discharges.
  • Ensure compliance with federal, state, and local regulations, including Department of Public Health (DPH) requirements.
  • Maintain a professional, respectful, and ethical work environment.
  • Exercise independent judgment and clinical decision-making within scope of LVN practice.
  • Provides day-to-day supervision and guidance to licensed and unlicensed nursing staff.
  • Supports staff education, coaching, and performance improvement as needed.
  • Does not include formal disciplinary authority unless delegated by management.
