About The Position

The position involves performing individualized nursing care for Subacute patients and their families under the direction of a Registered Nurse (RN). The role includes basic assessment data collection, participation in planning, execution of interventions according to the Plan of Care, and contribution to the evaluation of outcomes. Effective communication is essential to ensure that patients and families are educated about the Plan of Care and planned treatments.

Requirements

• AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - REQUIRED.
• California Licensed Vocational Nurse (LVN) - REQUIRED.
• Vocational Nurse IV Therapy/Blood Withdrawal Certification - REQUIRED.

Nice To Haves

• 1 Year CNA or LVN experience in a clinical setting.
• Experience with long-term care patients with complex needs.

Responsibilities

• Collect assessment data using observation, palpation, and auscultation per department guidelines.
• Document data collected and communicate it to the RN for analysis.
• Coordinate patient care activities with other healthcare team members.
• Implement the Plan of Care per department guidelines, providing specialized nursing care.
• Encourage patient and family participation in decision-making.
• Identify learning needs and barriers for patients and families.
• Collaborate with RNs in developing individualized teaching plans.
• Collect data indicative of patient/family response to care and document it.
