About the position

This position involves performing individualized nursing care for patients and families under the direction of a Registered Nurse (RN). The role includes basic assessment data collection, planning, executing interventions in accordance with the Plan Of Care, and contributing to the evaluation of outcomes. Effective communication is essential to ensure that patients and families are educated about the Plan Of Care and planned treatments.

Responsibilities

  • Collects assessment data using observation, palpation, and auscultation per Practice Act.
  • Documents data collected and communicates it to the RN for analysis and interpretation.
  • Coordinates patient care activities and workflow with other healthcare team members.
  • Implements the plan of care per department guidelines and collaborates with RNs for nursing decisions.
  • Encourages patient and family participation in decision-making and upholds individual/family rights.
  • Identifies learning needs of patients and families and collaborates with RNs to develop individualized teaching plans.
  • Updates written plan of care as directed by the RN and supports evaluation of care plans.

Requirements

  • AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association - REQUIRED
  • California Licensed Vocational Nurse (LVN) - CA Board of Vocational Nursing & Psychiatric Technicians - REQUIRED

Nice-to-haves

  • 1 Year LVN experience in an acute care setting or related area.
  • Vocational Nurse IV Therapy/Blood Withdrawal Certification - Various-Employee provides certificate - PREFERRED
