Palms Care Center is a family-owned skilled nursing facility located in Chowchilla, CA, with a capacity of 62 beds. We pride ourselves on fostering a positive workplace culture and are currently seeking a Licensed Vocational Nurse (LVN) to join our tight-knit team. This position offers the opportunity to start working immediately, with no prior experience required as we provide on-the-job training for new LVNs. A sign-on bonus is also available for new hires. The LVN will be responsible for ensuring the delivery of efficient and effective nursing care while achieving positive clinical outcomes and resident/family satisfaction in accordance with accepted standards of practice, state and federal regulations, and licensing requirements. The role operates within the scope of practice defined by the state Nurse Practice Act and involves overseeing resident care and directing nursing care during the assigned shift. This includes staff assignments, mentoring and educating nursing personnel, and collaborating with physicians and other medical professionals. The principal responsibilities of the LVN include conducting daily nursing functions in compliance with company, state, federal, and local regulations. The LVN will admit, transfer, and discharge residents while documenting care and services provided. Participation in the orientation of new residents and their families is also required, along with involvement in facility surveys and inspections. The LVN will ensure the delivery of high-quality care, positive clinical outcomes, and resident/family satisfaction during their shift. They will serve as a resource for all staff regarding the quality of services provided and will manage nursing personnel to limit overtime and agency personnel usage. The LVN will assess the work performance of nursing staff, conduct unit huddles for communication, and coordinate shift assignments and breaks for Certified Nurse's Aides. Additionally, the LVN will assume nursing administrative responsibilities during weekends and holidays as needed, ensuring compliance with the Minimum Data Set (MDS) process and that it reflects the resident's plan of care. They will also ensure that physician orders are followed, manage medication supplies, and promptly notify the attending physician and family of any significant changes in a resident's health condition. The LVN will document incidents, accidents, and grievances in accordance with company policies and federal/state regulations, investigate resident/family complaints, and take appropriate actions to resolve issues. The role requires knowledge of infection control protocols and the ability to evaluate the effectiveness of care interventions, identifying problems and trends to develop alternative interventions.
