About The Position

Position Summary: Concord Post Acute is seeking a dependable and skilled Licensed Vocational Nurse (LVN) to join our team on an on-call basis. The LVN will provide direct nursing care to residents, support clinical staff, and ensure high-quality patient outcomes in a fast-paced post-acute care environment. This role requires flexibility, strong clinical judgment, and a commitment to compassionate care. Requirements

  • Current, active LVN license in the state of California
  • Current CPR/BLS certification
  • Minimum of 1 year experience in a skilled nursing or post-acute setting preferred
  • Strong clinical and assessment skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently and adapt to changing patient needs

Responsibilities

  • Provide direct patient care in accordance with established nursing standards and care plans
  • Administer medications and treatments as prescribed by physicians
  • Monitor and document patient conditions, reporting changes promptly to the RN or physician
  • Assist with admissions, discharges, and transfers of residents
  • Collaborate with interdisciplinary team members to ensure continuity of care
  • Maintain accurate and timely documentation in compliance with regulatory requirements
  • Ensure a safe, clean, and supportive environment for residents and staff
  • Respond to call-offs and staffing needs on short notice
