About The Position
Position Summary: Concord Post Acute is seeking a dependable and skilled Licensed Vocational Nurse (LVN) to join our team on an on-call basis. The LVN will provide direct nursing care to residents, support clinical staff, and ensure high-quality patient outcomes in a fast-paced post-acute care environment. This role requires flexibility, strong clinical judgment, and a commitment to compassionate care. Requirements
- Current, active LVN license in the state of California
- Current CPR/BLS certification
- Minimum of 1 year experience in a skilled nursing or post-acute setting preferred
- Strong clinical and assessment skills
- Excellent communication and interpersonal abilities
- Ability to work independently and adapt to changing patient needs
Responsibilities
- Provide direct patient care in accordance with established nursing standards and care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and document patient conditions, reporting changes promptly to the RN or physician
- Assist with admissions, discharges, and transfers of residents
- Collaborate with interdisciplinary team members to ensure continuity of care
- Maintain accurate and timely documentation in compliance with regulatory requirements
- Ensure a safe, clean, and supportive environment for residents and staff
- Respond to call-offs and staffing needs on short notice
