Requirements
- Current Licensed Vocational Nurse (LVN) license in good standing
- Primary or outpatient setting healthcare experience
- BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care.
Nice To Haves
- 1–2 years of clinical experience in a primary care or outpatient setting
- Epic EMR
- Knowledge of Medicare Annual Wellness Visit requirements and preventive health screenings
- Microsoft Excel knowledge
Responsibilities
- The Medicare Wellness Coordinator LVN works in collaboration with the primary care provider and all members of the health care team.
- Primarily responsible for coordinating and conducting comprehensive Medicare Annual Wellness Visits (AWVs) and preventive health screenings under the supervision of a Primary Care Provider (PCP).
- This role ensures patients receive comprehensive preventive care services, including health risk assessments, screenings, education, and coordination of follow-up care.
- The LVN promotes a positive patient experience while maintaining compliance with Medicare guidelines and clinical protocols.
- This position requires expertise in the nursing process, using critical thinking skills to plan and coordinate care.
- Responsible and accountable for direct and indirect patient care for designated patient populations.
