Requirements

  • Current Licensed Vocational Nurse (LVN) license in good standing
  • Primary or outpatient setting healthcare experience
  • BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care.

Nice To Haves

  • 1–2 years of clinical experience in a primary care or outpatient setting
  • Epic EMR
  • Knowledge of Medicare Annual Wellness Visit requirements and preventive health screenings
  • Microsoft Excel knowledge

Responsibilities

  • The Medicare Wellness Coordinator LVN works in collaboration with the primary care provider and all members of the health care team.
  • Primarily responsible for coordinating and conducting comprehensive Medicare Annual Wellness Visits (AWVs) and preventive health screenings under the supervision of a Primary Care Provider (PCP).
  • This role ensures patients receive comprehensive preventive care services, including health risk assessments, screenings, education, and coordination of follow-up care.
  • The LVN promotes a positive patient experience while maintaining compliance with Medicare guidelines and clinical protocols.
  • This position requires expertise in the nursing process, using critical thinking skills to plan and coordinate care.
  • Responsible and accountable for direct and indirect patient care for designated patient populations.
