About The Position

The Licensed Vocational Nurse (LVN) identifies patient needs, maintains quality standards, and provides basic patient care. The LVN also partners with the Registered Nurse (RN) to perform skills, procedures, and other tasks that are outside their clinical scope of practice.

Requirements

  • High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
  • Completion of an accredited Vocational Nursing School program OR become licensed by equivalency required
  • BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
  • LVN - Current State of California Licensed Vocational Nurse License required
  • LVNIVTX - IV Therapy Certification (LVN) required within 180 days of hire or transfer into position
  • ACLS - Advanced Cardiac Life Support Certification preferredo For the following departments: Clovis: 02-61780 ICU Stepdown, 02-61792 Surgical CPCU. CHS HealthStream Course
  • BAC - Basic Arrhythmia Course required within 180 days of hire or transfer into positiono For the following departments: 04-87301 Centralized Flex Staffing, 04-87302 Centralized Float, 01-87300 Nurse Float-PR Only. Clovis: 02-87300 Nurse Float-PR Only. Refer to the department’s specific requirements, orientation skills checklists and competencies

Responsibilities

  • Use and practice basic assessment (data collection), participate in planning, execute interventions in accordance with the care plan or treatment plan and contribute to evaluation of individualized interventions related to the care plan or treatment plan.
  • Recognize, communicate and document changes in patient’s condition either emergent or in response to patient care.
  • Obtain and provide information on patients at change of shift briefings with nursing personnel and in contacts with physicians. Report on significant incidents involving nursing personnel, patients and others.
  • Contribute to the development and implementation of a teaching plan related to self-care for the patient.
  • Interact and appropriately utilize other members of the healthcare team to promote patient/family well being and support patient/family choices.
  • Administer medications to patients according to CMC policy following the 5 Rights of Safe Medication Practice. Observe and report adverse drug reactions to Pharmacy and RN.
  • Assure completeness of patient record.
  • Performs other job-related duties as assigned.
