Job Title: LVN/LPT Division/Program: San Bernardino: TAY Crisis Residential Treatment Starting Compensation: 30.00 USD Per Hour Working Location: San Bernardino, CA Working Hours/Shift: 8 Hour AM Shift Division/Program Overview: The TAY Crisis Residential Treatment (CRT) facility is a short-term, voluntary residential program for Transitional Age Youth (TAY) aged 18-25 experiencing a psychiatric crisis. It offers 24/7 services, including therapeutic and mental health care, substance use rehabilitation, pre-vocational or vocational counseling, medication evaluation and support, daily exercise, health and wellness education, and crisis intervention, all aimed at helping residents safely stabilize, recover, and transition back into the community. Placement within the grade is determined based on experience, internal equity, and other factors permitted by law. Requirements
- Applicants must have attained the age of 18 years of age
- Graduation from an accredited school of licensed vocational nursing or psychiatric technology required.
- One (1) year of medication administration experience is required.
- Valid California LVN or LPT license.
- Current CPR certification
- First Aid certification from persons qualified by agencies including but not limited to the American Red Cross.
Nice To Haves
- Associate degree, preferred.
- One (1) year of service experience treating the mentally ill in an inpatient setting, preferred.
Responsibilities
- The LVN / LPT provides direct client care including the administration of medications and supervising nursing activities performed by professional and para-professional nursing personnel on the unit.
- Duties include Ordering, preparing, administering, and documenting the administration of medication for clients according to physicians’ orders.
- Confirming delivery of all ordered medication from the pharmacy on shift worked.
- Communicating pertinent information verbally and in writing to all team members.
- Responsible for writing a weekly summary on the client as assigned.
- Monitoring and maintaining appropriate nursing documentation in the medical record for the shift worked.
