About The Position

The LVN/LPN position at Santa Fe Care Center is designed to organize and direct the duties of the assigned unit to ensure the delivery of quality nursing care to residents. The role is pivotal in maintaining compliance with Federal, State, Local, and company regulations while providing comprehensive nursing care. The LVN/LPN will be responsible for the total nursing care of residents during their shift, which includes administering prescribed medications and treatments according to approved nursing techniques, transcribing physician's orders, and charting progress notes that accurately reflect the care administered and the residents' responses. In addition to direct patient care, the LVN/LPN will conduct frequent rounds to evaluate the effectiveness of care delivery and maintain awareness of the comfort and safety needs of residents. They will also monitor the documentation completed by Certified Nursing Assistants (C.N.A.s) and facilitate accurate documentation in the residents' medical records. Coordination with therapists to ensure that residents' rehabilitation programs are met around the clock is also a critical function of this role. The LVN/LPN will be expected to participate in quality improvement activities within the department or unit and attend periodic in-services and staff meetings to enhance their skills and knowledge. This position requires strong communication skills to effectively interact with residents, family members, and all levels of the organization while maintaining confidentiality. The LVN/LPN will report directly to the Director of Nursing and is expected to be skilled in directing, motivating, and training staff, working collaboratively as part of a team.
