About The Position

The Licensed Vocational Nurse (LVN) position at San Jacinto Manor is responsible for providing quality nursing care to residents in an assigned unit. The role involves organizing and directing nursing duties, ensuring compliance with regulations, administering medications, and maintaining accurate documentation of care provided. The LVN will also participate in care planning and quality improvement activities, while ensuring the comfort and safety of residents.

Requirements

  • Current State of LVN/LPN license with a minimum of 2 years related experience or equivalent combination of education and experience.
  • Good standing in the state registry is required.

Responsibilities

  • Organizes and directs assigned unit duties to provide quality nursing care to residents.
  • Responsible for the total nursing care of residents in the assigned unit during the shift.
  • Explains procedures, treatments, and care plans to residents.
  • Administers prescribed medications and treatments according to approved nursing techniques.
  • Transcribes physician's orders and charts progress notes accurately.
  • Receives and gives report to the incoming/outgoing nurse and implements follow-up actions as necessary.
  • Conducts frequent rounds to evaluate the effectiveness of care delivery.
  • Prepares equipment and aids the physician during treatment and examination of residents.
  • Maintains awareness of residents' comfort and safety needs.
  • Observes residents, records significant conditions and reactions, and notifies supervisors or physicians as needed.
  • Takes vital signs to assess residents' conditions and detect deviations from normal.
  • Responds to emergency situations based on nursing standards and protocols.
  • Assists in the completion of care plans and participates in care conferences as necessary.
  • Reports and conducts investigations related to any alleged abuse to the DON/Administrator.
  • Ensures documentation reflects the care plan and that approaches are being followed.
  • Coordinates residents' rehabilitation programs with therapists to meet rehabilitation needs.
  • Identifies residents at risk for nutritional problems and implements preventative measures.
  • Records new or changed diet orders and forwards information to the Dietary Department.
  • Supervises and evaluates the implementation of the 'Resident's Rights'.
  • Participates in department or unit quality improvement activities.
  • Requests supplies and charges for items ordered through Central Supply.
  • Attends periodic in-services and staff meetings.
