About The Position

The Licensed Vocational Nurse/Practical Nurse (LVN/LPN) have a broad degree of practical nursing knowledge and skills. The LVN/LPN will care for adults psychiatric, medical, and geriatric patients at the Santa Rosa North VA Clinic.

Requirements

• U.S. Citizenship.
• Proficient in written and spoken English.
• Graduate of a school of practical or vocational nursing approved by the appropriate State agency.
• Active, full, current and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse.
• Completion of at least one year of qualifying experience at the GS-5 level or equivalent.

Nice To Haves

• 1-2 years of LVN/Specialty experience.

Responsibilities

• Perform retrospective medical review of non-VA health care records to determine medical necessity of visit.
• Review medical record documentation for requests for service (RFS) of authorized community care consults.
• Analyze submitted medical documentation to ensure all components necessary for medical record review are present.
• Determine test results by elevation of abnormal tests and report deviations to RN or MD/DO.
• Utilize knowledge of physical, social, and emotional factors when planning patient care.
• Apply knowledge of Medical Center Policies and Procedures to daily clinical functions.
• Promote staff involvement in the team environment for patient-centered care.
• Communicate a plan with the patient and/or caregiver and referring and accepting provider.
• Perform consult review, consult routing, and completion of consults.
• Manage indirect patient care including returning calls, responding to emails, and managing prescriptions.
• Apply the nursing process to systems or processes at the unit/team/work group level.
• Identify and assess resource utilization and safety issues.
• Participate in educational activities to improve clinical knowledge.
