About The Position

The Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) is responsible for overseeing the daily operations of nursing assistants and ensuring compliance with facility policies and regulations. This role involves administrative, clinical, and supervisory functions to maintain high standards of nursing care and support the overall well-being of residents in a long-term care facility.

Requirements

  • Current LPN/LVN license in the state of practice.
  • Experience in a long-term care or similar healthcare setting.
  • Strong knowledge of nursing procedures and regulations.
  • Excellent communication and interpersonal skills.
  • Ability to lead and supervise nursing staff effectively.
  • Proficient in charting and documentation practices.

Nice To Haves

  • Certification in geriatric nursing or related specialty.
  • Experience with electronic health records (EHR) systems.
  • Bilingual skills to communicate with diverse resident populations.

Responsibilities

  • Direct the day-to-day functions of nursing assistants in accordance with current rules and regulations.
  • Ensure compliance of nursing personnel with facility policies and procedures.
  • Review and recommend revisions to department policies and procedure manuals.
  • Plan shifts' services, programs, and activities with nursing staff and support personnel.
  • Maintain the Nursing Service Procedures Manual and ensure compliance by nursing personnel.
  • Make written and oral reports concerning shift activities as required.
  • Coordinate nursing services with other resident services for comprehensive care.
  • Participate in the development and implementation of the facility's quality assurance program.
  • Assist in planning the nursing services portion of resident discharge plans.
  • Interpret department policies and procedures to personnel and residents as required.
  • Admit, transfer, and discharge residents as necessary.
  • Complete accident/incident reports and maintain the Daily Census Report.
  • Perform administrative duties such as completing medical forms and charting as necessary.
  • Ensure confidentiality of resident information and report any violations.
  • Chart required documentation accurately and timely, including medication administration and treatment plans.
  • Prepare and administer medications as ordered by physicians, ensuring proper identification of residents.
  • Participate in employee performance evaluations and staffing requirements.
  • Provide leadership and training to nursing personnel on the unit/shift.
  • Ensure compliance with safety regulations and infection control procedures.
  • Monitor and maintain adequate supplies of medications and medical equipment.
