The LVN Nurse (PRN POOL) role is for a dedicated LVN passionate about long-term care and looking for a progressive career in healthcare, specifically within the rehabilitation arena. As a Charge Nurse, the individual will assess resident needs, develop individual care plans, administer nursing care, evaluate nursing care, and supervise Certified Nursing Assistants (CNAs) in the delivery of nursing care. This role also involves hiring, orientation, and monitoring nursing staff, training them, working closely with doctors, administering medications, and maintaining accurate documentation of treatment.

Requirements

  • Degree in nursing from an accredited Nursing program
  • LVN License, in good standing
  • 1 year of experience as a nurse
  • Effective communication skills
  • Ability to organize, prioritize and maintain confidentiality and react decisively in emergencies
  • Exceptional supervisory skills, taking a proactive role to supervising and developing nursing team
  • Excellent training skills, ability to provide hands-on training to nursing staff
  • Good problem solving and independent thinking skills
  • Case management skills, comfort with client relations

Nice To Haves

  • Experience in a long-term care facility preferred

Responsibilities

  • Supervising the nursing unit and coordinating the effective use of personnel, supplies, equipment and facilities
  • Reviewing staffing patterns and census of nursing units; reassigning personnel when necessary to ensure residents needs and budgetary controls are met
  • Managing and analyzing patient care following admission/readmission; conducting initial assessment for newly admitted or readmitted patients
  • Assisting in creating and implementing care plans that address individual residents' needs
  • Administering medications, IV therapy, gastronomy tube management as well as some wound care
  • Monitoring residents and communicating changes to physicians
  • Ensuring appropriate documentation of resident care and maintaining medical records to ensure safe, efficient and therapeutic care to residents
  • Troubleshooting and investigating complaints, taking necessary action to resolve problems as necessary
