About The Position

Prime Home Health is growing and busy! We are seeking a compassionate and skilled LVN working on PRN basis to join our home health team for in-person, in-home health patient visits in Castroville and surrounding areas. As an LVN on our team, you will play a vital role in providing comprehensive nursing care to adult home health patients. You will work closely with the interdisciplinary team to ensure the highest quality of care. This is not a remote from home position, you must live in the Castroville area in order to see patients in person.

Requirements

  • Valid LVN license issued by the State of Texas.
  • You must live in the Castroville or surrounding areas.
  • Minimum of one year of experience as an LVN providing in-home health patient visits.
  • Experience using Kinnser/WellSky EMR, (or others), efficient with apps such as Slack and others.
  • Current CPR BLS certification.
  • Strong clinical skills and knowledge of nursing principles and practices.
  • Excellent communication and interpersonal skills to interact effectively with patients, families, and the interdisciplinary team.
  • Professional, compassionate and sensitive to the physical and emotional needs of our patients and their families.
  • Ability to work independently and as part of a team, demonstrating flexibility and adaptability in a dynamic healthcare environment.
  • Valid driver's license and reliable transportation for home visits.
  • Commitment to maintaining confidentiality and upholding ethical standards in patient care.

Responsibilities

  • Provide direct patient care in accordance with the established plan of care and individualized patient needs, including administering medications, performing treatments, monitoring vital signs, and managing symptoms.
  • Conduct thorough assessments of patients' condition, document observations, and maintain accurate and up-to-date medical records, including progress notes, medication administration records, and treatment plans.
  • Educate patients and their families about their condition, treatment options, and end-of-life care, providing emotional support and guidance to help them cope with the challenges they may face.
  • Maintain open and effective communication with the interdisciplinary team, patients, families, and caregivers, ensuring the coordination of care, addressing concerns.
  • Adhere to infection control protocols, maintain a safe environment for patients and staff, and comply with all relevant laws, regulations, and organizational policies and procedures.
  • Stay updated with the latest developments in hospice care, attend educational programs, and participate in ongoing professional development activities to enhance knowledge and skills.
