About The Position

We’re excited to announce an opening for a Licensed Vocational Nurse (LVN) Liaison! This is an in-office position based at our Chico headquarters, with a primary focus on hospice referrals and a secondary role supporting home health referrals. The LVN Liaison will also meet with patients and families in person to provide information about our services and help guide them through the referral process.

Requirements

• Current California LVN license (required)
• Hospice or palliative care experience strongly preferred; home health a plus
• Strong communication and interpersonal skills with a compassionate approach
• Organized, detail-oriented, and proficient in electronic health records

Responsibilities

• Process hospice referrals (primary) and support home health referrals (secondary)
• Provide in-person informational visits to patients and families regarding hospice philosophy and services
• Collaborate with physicians, hospitals, facilities, and our internal teams to ensure smooth transitions of care
• Maintain accurate documentation and support care coordination across the interdisciplinary team
