About The Position

Seva Home Health is looking for an LVN for our growing Home Health business serving Tulare County and surrounding communities. Mileage is reimbursed and driving time is paid. We are a highly-rated, locally-owned and operated agency with compassionate health professionals serving the communities we live in.

Requirements

2 years LVN experience, home health preferred
LVN license, required
IV Therapy/Blood Draw certification required
Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook)

Nice To Haves

Experience with Kinnser (WellSky) EMR preferred
Experience with Allscripts preferred

Responsibilities

Serving as a point of contact throughout the admissions process
Ensuring coordination of care with other team members and marketing team
Communicating effectively with patients, family members, case managers, healthcare providers and co-workers
Providing education to patients and family members regarding their home healthcare experience
Providing an outstanding admissions experience for our patients
Receive and appropriately document new referrals in a timely manner.
Participate in coordinating care with Nurse Supervisors and patient interdisciplinary team.
Provide effective communication to patients, staff members, other health care professionals and referral sources.
Respond to agency and patient needs in a professional and creative manner.
Demonstrate commitment, professional growth and competency.
Understands and applies a smooth and efficient admissions process and an outstanding experience for our customers
Is able to multitask and has excellent organizational skills
Has outstanding communication skills (written, verbal and non-verbal)
Autonomous and self-motivated
Demonstrates professionalism in all interactions
