LVN Discharge Planner - Transitional Care PD Day
About The Position
LVN Discharge Planner at Providence Little Company of Mary Torrance in Torrance, CA. This position is Per Diem and will work 8-hour, Day shifts. We are also proud to be included in the Maternity Care Honor Roll by the California Surgeon General for 2024 The Care Management Assistant (CMA) collaborates with the patient/family, the acute multidisciplinary team, and post-acute providers to implement the patients discharge plan. The CMA also proactively escalates risks and barriers to discharge to the Case Manager; documents all discharge planning efforts in the patient's medical record in a timely manner; maintains ongoing communication with the multidisciplinary team and follows all regulatory requirements and ministry polices regarding care coordination/discharge planning. Requirements
- Upon hire: California Licensed Vocational Nurse.
- 1 year Healthcare related position.
