About the Role We are seeking an experienced and compassionate LVN Clinical Coordinator to join us in this mission. The LVN Clinical Coordinator is responsible for ensuring that patient care is coordinated and managed appropriately as well as is responsible for ensuring that care and services are delivered appropriately.

Requirements

  • Valid California LVN license
  • Minimum 2 years of hospice experience
  • Current CPR certification
  • Valid driver’s license and insured, reliable vehicle

Nice To Haves

  • Experience with Home Care Home Base (HCHB) preferred

Responsibilities

  • Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.
  • Ensures seamless transition of patients to home care by providing direct oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
  • Reviews clinical documentation to help determine Hospice appropriateness.
  • Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
  • Tracks and updates team members on new hospitalizations, hospital holds and resumptions of care.
  • Communicates with patients, families and caregivers regarding Hospice services.
  • Coordinates with physicians and referral sources (as appropriate) regarding Hospice plans of care.
  • Assists in ensuring that patients are seen in a timely manner and works with physicians to receive updated start of care orders when necessary.
  • Cross trains to provide back-up coverage for scheduling and intake coordination when needed.
  • Filter calls from field staff as well as help with questions to support field staff, help with stat or urgent needs, Order medications.
