About The Position

The LVN Charge Nurse position at the Burbank Congregate Living Center involves providing direct nursing care to medically fragile sub-acute level patients in a congregate living health facility. The role includes implementing care plans, documenting patient progress, and collaborating with an interdisciplinary team to ensure effective patient care. This full-time position requires working 12-hour shifts, either during the day or night.

Requirements

• Current CA LVN license
• CPR certification
• Minimum of 2 years nursing care experience in sub-acute or congregate living health facility settings preferred
• Vent/Trach Certification preferred or can be obtained upon hire
• IV Certification preferred but not required

Responsibilities

• Perform direct nursing care including AM/PM care.
• Implement the plan of care initiated by the registered nurse.
• Provide accurate and timely documentation consistent with the plan of care.
• Participate in patient and family education, reporting identified needs to the Director of Nursing (DON).
• Utilize available resources in planning care and consult with RNs, medical staff, and outside specialists as needed.
• Continuously review, evaluate, and document the effectiveness of nursing care and client goals for discharge.
• Use equipment and supplies effectively and efficiently.
